all information requested. Failure to do so will result in disqualification.

except editorials. Clumns may have two entries if each represents a separate topical
issue. To submit two issues for Columns, you must supply 3 entries on one subject matter
from one publication, and three entries covering another subject matter.

- Entries must be published works dated between June 1, 2001 and May 31,
2002
- Entries must be copies of the original (with the exception of Internet and
other electronic media which must include an Internet address and date of posting
or copyright date)
- Writers' names, as well as any reference to the name of the publication
(including logos), must be blacked out or deleted on all copies (with the exception of
books).
- Two copies off each entry must be submitted (except books, which require only one
copy).
